An Ontario Student Transcript (OST) is an official and legal record of a student’s achievement at the secondary school level in Ontario. It includes diploma and/or certificate information, as applicable. All credits accumulated at secondary schools in Ontario will appear on one OST. The OST is retained by the last secondary school (or its board) that you attended.
Post-Secondary Applicants: Retired students applying to Ontario colleges/universities should use ontariocolleges.ca or ouac.on.ca for electronic transfer.
Retired / Graduated Students less than 5 years: Must complete this form: Transcript Request Form (Left HC less than 5 years ago)
Retired/Graduated Student more than 5 years : Must complete the Records Request Form: Transcript Request Form (has left HC more than 5 years ago).
We cannot release any student information over the phone.
We cannot process a transcript request if the last secondary school you attended was not with the Peterborough, Victoria, Northumberland and Clarington Catholic District School Board.
Student records are confidential. You will be asked for proof of ID prior to the release of the requested transcript.
